Most TMJ pain settles with a properly fitted stabilisation splint, physiotherapy and lifestyle change — invasive treatment is reserved for the minority whose symptoms do not respond. Where imaging (MRI is the investigation of choice) shows internal derangement or degenerative change, arthrocentesis and arthroscopy are effective minimally invasive options. Total joint replacement using a custom-made prosthesis is reserved for advanced arthritis, ankylosis, or failure of previous surgery, and is planned from a CT scan on a virtual model.
